Payment & Insurance
Andrea is a Licensed Clinical Social Worker in the state of Wisconsin and, therefore, you may be able to receive some reimbursement for the services she offers depending on your current health insurance plan. Regardless of whether you choose to use out-of-network benefits, health savings account (HSA) or self-pay, payment is made in full at the time of your session. A receipt of payment with the required coding will be provided if you would like to submit a claim to your insurance company for reimbursement. Please contact your insurance provider to verify how your plan compensates you to determine your financial responsibility for psychotherapy services .
Questions to ask insurance company:
Do I have out-of-network coverage for mental health?
What is my deductible for out-of-network coverage for mental health? How much has been met so far?
Do I need pre-authorization in order to begin outpatient therapy services?
What is the coverage amount per therapy session?
How many sessions per year does my health insurance cover?
Is there any other information that is needed to obtain reimbursement? (The information provided to you on the receipt of service will include: Therapist’s name, credentials, office address/phone, federal tax ID number; date(s) of service, CPT code(s), diagnostic code, and fee for service.)

60 min. session: 90837
45 min. session: 90834

Good Faith Estimate:
Under the Federal No Surprises Act (H.R. 133 – effective January 1, 2022), you have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
